How much does Jardiance cost with Medicare in New Jersey?
Jardiance has a Medicare negotiated price of $197 for a 30-day supply in 2026, which comes to about $1,052 for the year under the standard Part D design if it is your only drug, and New Jersey Part D members paid an average of $31.71 per 30-day fill in 2024.
Jardiance (empagliflozin) is an SGLT2 inhibitor: it helps the kidneys pass extra glucose in the urine. CMS lists three conditions it commonly treats: type 2 diabetes, heart failure and chronic kidney disease. That is why a cardiologist, a kidney specialist or a family doctor may each prescribe it; 8,605 New Jersey prescribers wrote it for 59,377 Part D members in 2024.

Why Jardiance is on so many medication lists
Because it treats three common conditions, Jardiance reached 1,883,000 Part D enrollees and $8.8 billion in gross spending nationally in 2023, according to CMS. The negotiated price trims 66% off the $573 list price. National Part D data show average gross spending per empagliflozin claim of $1,091.19 in 2025 and $386.83 in the first quarter of 2026.
No generic empagliflozin yet
No empagliflozin entry appears on the FDA First Generic Drug Approvals lists for 2024, 2025 or 2026, the last checked through its October 5, 2026 update. Its close relative Farxiga received first generic approvals in April 2026, but that does not make empagliflozin interchangeable with dapagliflozin; switching between them is a prescriber decision. CMS lists no removal for Jardiance, so brand Jardiance keeps its negotiated price into 2027.
Jardiance on its own, and stacked with other brands
On a plan with the $615 deductible and 25% coinsurance, the first three fills cost $197 each, April costs $67.25, and the remaining 8 months run $49.25 apiece: $1,052.25 for 2026. For 2027, with a $700 deductible, the same pattern totals about $1,116.
Many people taking Jardiance also take a blood thinner. Add Eliquis at its $231 negotiated price and the 2026 total for both is about $1,745.25. One more brand-name drug could push you to the $2,100 limit, after which covered Part D drugs cost $0 for the rest of the year.
Lowering what you pay
Extra Help removes the deductible and caps brand-name copays at $14.40 per fill for 2027; twelve fills would be $172.80. The 2026 limits are $23,940 of income and $18,090 of resources for a single person. Medicare.gov also suggests asking the drug maker whether it offers help, and a plan with a $0 drug deductible changes the early months more than anything else.
Questions to bring to a Jardiance plan review
Bring the strength printed on your bottle, since Jardiance comes as 10 mg and 25 mg tablets. Then ask each plan four things: which tier holds Jardiance, whether step therapy means trying metformin or another drug first, whether a 90-day supply through a preferred or mail-order pharmacy lowers the monthly share, and whether your usual pharmacy is preferred or merely in network.
New Jersey totals give a sense of scale: 486,565 thirty-day fills in 2024, $270,796,270 gross, of which members paid $15,428,606.
Where the deductible changes everything
In the 2027 Camden County file, 3 of the 34 Medicare Advantage plans with drug coverage have a $0 drug deductible. On a plan like that, with 25% coinsurance and the $197 price, twelve Jardiance fills would run $591, against roughly $1,116 on a plan using the full $700 deductible. Real plans use tier copays, so treat this as a direction, then price the actual formulary.
Formulary rules moved too: 862 renewing plans nationwide added prior authorization for Jardiance for 2027 and 155 added step therapy, per CMS formulary files, so read the 2027 drug list even if your plan covered it without conditions in 2026. If your income sits just above PAAD, New Jersey's Senior Gold program (2026 income $54,943 to $64,943 single) charges $15 plus 50% of the remaining cost of each covered drug, then a flat $15 once your out-of-pocket spending passes $2,000 for a single person.
