How much does Farxiga cost with Medicare in New Jersey?
Farxiga carries a Medicare negotiated price of $178.50 for a 30-day supply in 2026, about $997 for the year under the standard Part D design if it is your only drug, and FDA approved the first generic dapagliflozin tablets in April 2026; New Jersey Part D members paid an average of $30.12 per 30-day fill in 2024.

Brand Farxiga or generic dapagliflozin in 2027
FDA announced on April 7, 2026 that it had approved the first generic versions of Farxiga tablets for several companies; its first generic list shows 15 single-ingredient dapagliflozin applicants dated April 6, 2026, plus generic versions of the dapagliflozin and metformin combination. FDA tells patients to contact manufacturers about availability.
CMS has not listed Farxiga as removed from the negotiation program as of October 2026; a drug leaves only after a generic is approved and marketed, and CMS publishes that determination. Until then the brand keeps its negotiated price. On your 2027 plan, look for both names, brand Farxiga and dapagliflozin, and compare tiers.
How the 2026 price came about
CMS says 994,000 people with Part D used Farxiga in 2023, with $4.3 billion in gross costs. The negotiated $178.50 sits 68% below the $556 list price. Nationally, average gross spending per Farxiga claim went from $997.75 in 2025 to $328.97 in early 2026.
Worked example on a standard plan
Take the $615 deductible and 25% coinsurance from Medicare.gov. Your first three fills are $178.50 each, the April fill is $104.25, and the last 8 fills cost $44.63. That is $996.75 for 2026, well below the $2,100 cap. In 2027 the deductible can reach $700, which puts the same year near $1,060.50.
A generic on a lower tier, or a plan with a $0 drug deductible, would cut that further. Ask before you assume the brand is your only option.
What does not work with Medicare, and what does
The Farxiga savings card is not available to anyone enrolled in Medicare Part D, even paying as a cash customer. Extra Help is: 2026 limits are $23,940 income and $18,090 resources for one person, with brand copays of no more than $14.40 in 2027. The Medicare Prescription Payment Plan can also turn the deductible months into smaller monthly bills. New Jersey residents who qualify for PAAD pay $7 for a covered brand prescription and $5 for a generic, so the generic saves $24 over twelve fills even there.
Farxiga in New Jersey, and on 2027 drug lists
Statewide, New Jersey Part D members filled Farxiga about three times for every four Jardiance fills. Pharmacies dispensed 350,982 month-equivalents in 2024, billed at $188,431,614; enrollees covered $10,572,337 themselves, close to $238.71 apiece.
The generic is already showing up in plan design. For 2027, 2,368 of the 4,748 renewing Part D plans nationwide put dapagliflozin on a lower tier than in 2026, the largest shift of any drug on these pages, while 1,613 added step therapy, per CMS formulary files.
The April 2026 FDA list names the companies cleared to make generic dapagliflozin, among them Teva, Sandoz, Zydus, Cipla, Lupin, Aurobindo, Biocon, Alembic, Alkem, Macleods, Ajanta and Micro Labs. The same list adds generic versions of Xigduo XR (dapagliflozin with extended-release metformin) and of Qtern (dapagliflozin with saxagliptin), so if you take a combination tablet, ask about that exact product rather than plain dapagliflozin.
Prescribed for kidney disease?
If a kidney specialist started you on Farxiga for chronic kidney disease rather than for diabetes, say so at the pharmacy counter. Generic labels can list fewer uses than the brand label, and the April 2026 FDA notice described the newly approved generics in terms of glycemic control and heart failure hospitalization in type 2 diabetes. Your nephrologist and pharmacist can confirm whether substituting the generic suits your diagnosis, and your plan can tell you whether brand Farxiga needs an exception.
