Why so many plans have a $0 premium

Medicare pays each Advantage plan a monthly amount for every member, and many carriers set the plan premium at $0 and recover costs through copays, networks and the out-of-pocket maximum. In Camden County the remaining 17 plans charge a premium, the highest at $149 a month for 2027.

7 of the $0 plans have no drug coverage at all. Those suit people with VA or other creditable drug coverage; anyone else would need to think about the Part D late enrollment penalty.

What separates one $0 plan from another

The network, the drug list, the out-of-pocket maximum and the extras. In-network maximums on Camden County plans run from $4,150 to $9,850 for 2027. CMS published 2027 star ratings on October 8, 2026, but our data snapshot does not hold them yet, so this page uses each plan's 2026 rating: 10 of the 34 returning plans carry a 2026 star rating of 4 or higher. Check a plan's 2027 rating on Medicare.gov Plan Finder.

The choice also shrank a little: Camden County had 44 plans and 27 with a $0 premium in 2026, against 41 and 24 for 2027.

Worked example: $0 premium against a lower cap

Two UnitedHealthcare HMO-POS plans in the 2027 Camden County file show the trade. One charges a $0 premium with a $7,150 in-network cap. The other charges $115 a month, $1,380 a year, with a $6,900 cap. In a quiet year the $0 plan comes out $1,380 ahead. Even in a year with a surgery that hits both caps, the premium plan costs $8,280 against $7,150, so a lower cap is worth paying for only when the gap between caps is bigger than the extra premium.

Either way, the $202.90 Part B premium, $2,434.80 over 2026, comes out of your Social Security first. Humana and UnitedHealthcare field the most $0 plans in the county for 2027, counting plans with and without drug coverage.
